Can I modify the store layout?

Sure you can. The cart use two scripts as the header and footer templates of the store (header.asp and footer.asp).

How can I set up a database DSN?

First of all create a DSN in your server, after that open the script settings.asp and find this line ' pDatabaseConnectionString = "DSN=comersus".
Remove the comment and replace the name comersus with your DSN name.
Then comment your previous database connection string and save the script.

If I change the layout and the I buy one of the Power Pack will I loose all the changes?

No, the Power Packs is just an add-on that will add files and folders without modifying your current templates.

There is nothing wrong with your store. That means that you are not charging an extra amount for using that payment method. That amount is not related to the amount being paid but to the extra amount related to that payment method.

My Real Time UPS Rates are too high - or too low -.

UPS Rates are obtained from UPS Site, so you are comparing with different information. If you are checking the rate at UPS use: Residential address, same day, Daily Pickup, 1 package, bill to UPS account, Your Package (and make sure that Residential, Your Package, Daily and weight are the same in the store)
